Fill out the pre-screener below to see if you might be eligible. If you meet the requirements, Bento will submit your application to IEHP for their approval.

IEHP will determine your eligibility

Once you apply, IEHP will determine your eligibility, and will require clinical documentation to support your application. For more info on who’s eligible, click here.

If IEHP determines that you are eligible, Bento will notify you with instructions on how to start ordering!

In addition, you are eligible to receive free health coaching from a Bento nutrition expert. You’ll work one-on-one with a coach who will help you set goals, stay motivated, and make the most of your grocery orders.

Answer the questions below to determine if you could qualify for Bento. If so, we will let you know what your next steps are.

Who is eligible?

Bento is free for eligible Inland Empire Health Plan members in Riverside County and San Bernardino County who meet eligibility criteria and also have a clinically-documented, nutrition-sensitivehealth condition, recent hospital discharge or risk of hospitalization, and extensive care coordination needs.

Approval for initial 12 weeks of this service is subject to IEHP review, and requires clinical documentation of your condition. Members who previously received this service may not be eligible. After 12 weeks, continuation of services will be dependent on the determination of medical necessity (including clinical documentation), will need to be approved by a Registered Dietitian, and is subject to IEHP approval.

All it takes is your phone number to start. Please note that it may take up to 2 weeks for Inland Empire Health Plan to determine your eligibility after you apply.

Note: Qualification is not impacted by immigration status, employment status, or nationality. If you are a member of Inland Empire Health Plan and have a health condition, you can apply.

FAQ

  • Inland Empire Health Plan members with a clinically-documented, nutrition-sensitive health condition (e.g diabetes, high blood pressure, etc) or recent hospital discharge.

    Qualification is not tied to immigration or employment status, nationality, or any other factor other than your insurance provider and health state.


  • This is a service provided by Inland Empire Health Plan to improve your overall health.

  • It may take up to 2 weeks for Inland Empire Health Plan to determine your eligibility after you apply.

  • Inland Empire Health Plan requires documentation of your health condition within the past 60 days to be eligible for this benefit.

    Here are examples of qualifying documents that are needed:

    • Hospital discharge paperwork

    • Primary care physician paperwork 

    • Prescription receipts from the last 60 days relevant to your condition

    • Skilled nursing facility discharge papers that pertain to your condition
