New Requirements From Kern Family Health Care

To Receive Groceries From Bento

Kern Family Health Care has changed the eligibility requirements to receive Medically Tailored Food. To receive Bento groceries, your healthcare provider needs to submit a referral on your behalf. Only members with certain health conditions are eligible.

Am I eligible?

If you have one or more of the following health conditions, you may be eligible for Bento: 

Asthma

Cancer

Cardiovascular Disorder

Chronic Kidney Disease

Chronic or Disabling Mental Health Disorders

COPD

Dyslipidemia

Elevated Lead Levels

End-Stage Renal Disease

Fatty Liver

Gastrointestinal Disorders

Gestational Diabetes

Heart Failure

High Cholesterol

High Risk Perinatal Conditions

HIV

Hypertension

Liver Disease

Malnutrition

Obesity

Stroke

How do I sign up?

Contact Your Provider

You may contact a primary care doctor, nurse practitioner, registered nurse, enhanced care manager, specialist, or physician assistant.

Get Referred

Ask your provider to refer you to Bento using Kern Family Health Care’s provider portal.

Important: Be sure to have your provider clearly indicate on the referral that you want to receive Bento.

Order

If you are eligible, Bento will send you a text with instructions on how to schedule your free grocery deliveries.

FAQs

  • Initially you will be approved for 6 weeks, but your Bento services can be extended for as long as your provider indicates that it is medically necessary. Remember to check in with your provider regularly so that they can continue requesting Bento groceries from Kern Family Health Care on your behalf.
